The following terms and conditions apply to the Brand New Day Rewards Plus Program ("the Program"). The Program is available where law and regulation allow. Please read these Terms and Conditions carefully. The Program’s Terms and Conditions are subject to change at any time. As used in these Program Terms and Conditions, the words "you" or "Rewards Member" mean any member eligible for the Program. The words "we," "us" and "our" refer to Universal Care, Inc., including its subsidiaries, affiliates, agents or administrators servicing the Program (hereinafter referred to collectively as Brand New Day).
Eligibility:
Rewards Members must:
- Be an active Brand New Day Medicare Advantage Plan member.
Earning Rewards:
All Rewards Members are eligible to earn the following rewards:
- Health Risk Assessment = earn $25
- Annual Wellness Visit = earn $50
Check with your doctor to see if you are due for any of the below screenings:
- Diabetic retinal Exam = earn $10 (completed with an Optometrist or Ophthalmologist)
- Mammogram = earn $30
- Colorectal cancer screening (one of the below) = earn $25
- Colonoscopy
- Sigmoidoscopy
- Fecal Occult Blood Test
- Flu shot = earn $10
Your eligibility for screening rewards will vary based on your doctor's recommendations. You can only earn rewards for the screenings you are due for.
- Each of these rewards can only be earned once in 2023.
- Rewards will not be fulfilled outside of your plan year; 2023 rewards will be issued until 12/31/2023.
- No rewards exceptions will be issued after 1/31/2024.
- Health Risk Assessment can be completed prior to your effective date; the reward will not be fulfilled until after your effective date. All remaining rewards will not be fulfilled for activities completed prior to your plan effective date.
The rules for earning rewards are administered by Brand New Day Plan. We reserve the right to modify, amend, or terminate rules and/or the ability to earn rewards at any time with or without notice. We are solely responsible for all rewards eligibility standards and determination of the Rewards Card issuance.

Reward Payment:
Once a reward activity is completed, Rewards Members will receive a prepaid reloadable Visa card (the “Rewards Card”) sent to the address on file. This is the Rewards Card for the entire duration of the Program. As Rewards Members complete additional activities that earn rewards, additional funds will be added to the Rewards Card initially received. After completing the first reward activity, Rewards Members can expect their Rewards Card to arrive within 60 - 90 business days. Please keep it in a safe place. We are not responsible for lost or stolen cards. Please do not throw away your card when you have spent the balance.
How to Use your Reward Card:
You may be required to activate your Rewards Card before first use following the instructions on the card.
Rewards dollars can be spent at:
- Grocery Stores, Supermarkets
- Convenience Stores, Markets, Specialty Stores, and Vending Machines
- Bakeries, Eating Places and Restaurants
- Made-for-Me Meals program
- Electronics Sales
- Household Appliance, Building Material, Hardware, Lumber and Home Supply Stores
- Florists Supplies, Nursery Stock and Flowers
- Paints, Varnishes, Glass, Wallpaper and Supplies
- Lawn and Garden Supply Stores
- Fuel Dispenser, Automated
Funds cannot be spent at big box retailers/discount stores, wholesale clubs, pharmacies or used to purchase alcohol, tobacco products, and firearms or to receive cash at checkout or from an ATM.
Reward Expiration:
You have until 12/31/2023 to complete your eligible rewards activities. Rewards funds expire when you leave your Brand New Day plan or on 12/31/2023.
